Duplicate Passport in lieu of
lost/damaged

document from Village or Block or District Authorities to prove
the change of address. The document should be duly attested by the
Home Department, General Administration of the concerned State and
counter attested by Consular section, Ministy of External Affairs,
CPV Division, Patiala House, Tilak Marg, New Delhi.

Change of Name
>
Application
form (EAP 1) duly filled and signed along with 4 photograph
to be submitted along with:

Cuttings
of two leading newspapers (one in India and the other in Portugal)
carrying advertisement notifying the change of name and asking for
any objection thereof to be reported to Embassy of India within
30 days of the publication.
